Program provides: Clubhouse – a program individuals can attend 1- 5 days/week based on a model in which the participants, or “members,” come to work, learn, socialize, refine social skills, improve confidence and self-esteem, participate in volunteer activities & support one another.
Community based Case Management assists clients with accessing existing community resources, planning and coordinating services, and monitoring the services and goals that have been put in place. Provided for adults in the city of Charlottesville, Counties of Albemarle, Fluvanna, Greene, Louisa, and Nelson. Residents of Buckingham County are handled on a case by case basis. For adults.
